Blumenthal manual small incision cataract surgery (SICS) is an established procedure for cataract surgery, especially in the developing world. This video shows Blumenthal manual SICS in a difficult situation i.e. hypermature morgagnian cataract. Blumenthal manual SICS was done without single use of any viscoelastics. The difficulty faced, mainly in performing a large capsulorhexis in absence of cortical support, is demonstrated in the video. The video also demonstrates the use of sheet glide as an alternative to prolapse the nucleus. The details of the technique would be presented in the video.

This video film on the Manual Small Incision Cataract Surgery (MSICS) has an introduction by Prof. Michael Blumenthal, inventor of the Blumenthal Technique of  MSICS and is presented by Prof Ravi Thomas, Program Instructor for the L V Prasad Eye Institute-MSICS Program.


At the outset, the Blumenthal Technique is illustrated in an immature cataract. In the next section, every step of the Blumenthal technique is separately demonstrated in a case of white cataract, brown cataract, advanced immature cataract and hypermature cataract. In the third and final section, the complete (unedited) surgeries of advanced immature cataract and white cataract are presented.

This video illustrates surgical technique of sutureless extracapsular cataract extraction and posterior chamber intraocular lens implantation. After making a fornix based conjunctival flap, frown incision is given and sclerocorneal tunnel is made. Capsulorrhexis, hydrodissection and hydrodelineation are performed. Nucleus is delivered into anterior chamber and then out of tunnel with aid of viscoelastic and irrigating vectis. After cortical aspiration, lens is implanted. As tunnel architecture offers good wound integrity, this self-sealing wound is covered with conjunctiva. This manual technique is safe, cost effective and has postoperative result as good as after phacoemulsification.
