Evaluation of Congenital Ptosis

Congenital ptosis is a condition commonly seen by an ophthalmologist. The array of examinations and available choice of surgical procedures taught theoretically often serves only to confuse. In this video, we demonstrate the evaluation of congenital ptosis, the techniques of the most relevant measurements, and the selection of the appropriate treatment based on these measurements. A surgical procedure for a patient with minimal to mild ptosis needs to produce accurate outcomes. Performed on the appropriate patient, the Fasanella-Servat is the most predictable of the procedures known for ptosis correction; the tarso-mullerectomy gives a correction by an amount exactly half the excised height. In this video, we demonstrate the steps of a modified Fasanella-Servat procedure; this procedure, in our hands, is performed with ease, and results in consistent and satisfactory outcomes.

Resection of the levator palpebrae superioris for correction of ptosis is a procedure that is applicable to a wide range of severity of ptosis, and results in a natural appearance. A familiarity with the anatomy with the various layers of the eyelid, and an understanding of the length to be resected in the particular patient, are essential for success. We take you through the steps of the LPS resection in this video, taking special care to demonstrate the anatomic landmarks and principles one needs to adhere to, while avoiding the possible pitfalls.

Suspension of the eyelid to the frontalis muscle is the procedure of choice in severe congenital ptosis with poor levator action. In this video, we demonstrate the surgical procedure utilizing a commercially available elastic silicone rod; the procedure is convenient and effective, and at the same time circumvents the disadvantages of other known tarso-frontal sling procedures.

Myogenic ptosis, caused by the diseases of the extraocular muscles, may be caused by multiple disease entities. The first step to successful management of myogenic ptosis is the evaluation of these conditions. Two conditions commonly causing myogenic ptosis are myasthenia gravis, and chronic progressive external ophthalmoplegia. Both the conditions also have ramifications beyond the eye alone. In this video, we demonstrate the clinical features, signs to elicit, and additional evaluation in these two conditions.
