This training video on sutureless vitrectomy illustrates the steps of this safe and feasible technique for a variety of retinal disorders. While it is already proven to be effective and advantageous in certain conditions, sutureless vitrectomy probably requires more modification and innovation of instruments as well as experience to further its use. The greatest advantage is felicity of use in paediatric cases, the immediate post-operative recovery time, decreased entry-exit time, good cosmetic appearance and decreased post-operative inflammation.

This new and innovative surgery demonstrates the preparation and use of dye to stain the vitreous and the ILM or internal limiting membrane. Staining with Sodium Fluorescein dye for clear vitreous is demonstrated in idiopathic macular hole surgery. A second surgery demonstrates the use of Indocyanine Green to stain the ILM in a case of macular hole secondary to vasculitis.

A patient with proliferative diabetic retinopathy with pre-macular fibrosis does well with surgical intervention. A healthy retina underlying the area of dense pre-macular fibrosis is the main reason for good outcomes in this group of patients. In this surgical video, we demonstrate the procedure on a patient with complex pre-macular fibrosis with poor pre-operative visual acuity. A clear dissection of the membrane is achieved with minimal bleeding when the plane of dissection is good.
