The Root Cause Analysis (RCA) Workshop held on 10th September 2025 brought together quality leaders from across the LVPEI network for an intensive, hands-on learning experience. Led by Mr Saravanan Saibaba, a seasoned trainer with over 30 years of experience in eye care, the session focused on equipping participants with practical tools to identify and resolve systemic issues. Attendees included Heads of Campuses, Quality Faculty HODs, Level 2 members, and key representatives from the KAR campus.The workshop blended presentations with group exercises, allowing participants to apply techniques such as flow chart mapping, Fishbone analysis, the 5 Whys method, and Pareto analysis. These tools helped teams drill down to root causes and prioritize corrective actions. A key highlight was the emphasis on developing CAPA plans with measurable KPIs to ensure accountability and track progress. The session reinforced the importance of structured incident review and collaborative problem-solving in driving sustainable quality improvements.
